AO American Benefits, powered by USABG, offers private health insurance plans through licensed agents. This policy explains what we collect on an advisor quiz and how we use it.
What we collect
- Your ZIP code, or your business ZIP code
- Who needs coverage (household type), the ages of the people on the plan, and the number of children
- For business inquiries, the number of employees who need coverage
- What you pay each month for coverage now, or that you don't currently have coverage
- Whether you receive a Marketplace subsidy
- A general health answer
- A health checklist of medical conditions, treatments and medications (for example diabetes, cancer, heart attack or stroke, liver, kidney or heart disease, autoimmune disorders, planned surgery, specialty or biologic medications, and diabetes or weight-loss injections), or that none apply
- Your annual household income range
- Your first and last name, phone number and email address
- Your contact consent, and the time it was given
We save quiz answers as you go, including on a quiz you don't finish, so an advisor can follow up on your request.
How we use it
Your answers are sent to the advisor's customer system so a licensed agent can review private health plan options and contact you about your request. We use your health and income answers only to understand which plans may fit and to prepare for that conversation. We do not sell your information.
Contact consent
The quiz has one consent checkbox. Checking it means you agree to be contacted by AO American Benefits by phone call, text message and email at the number and address you provide, about coverage options and offers (marketing) and about appointments, scheduling and your coverage inquiry (non-marketing). Calls and texts may use automated technology or prerecorded messages. Consent is not a condition of purchase. Message frequency varies. Message and data rates may apply. Reply STOP to any text to opt out, or HELP for help. You can also ask us to stop calling or emailing at any time.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information may be shared with vendors that support text messaging, such as a customer-service platform. All other use of information excludes text messaging originator opt-in data and consent; this information will not be shared with any third parties.
